I'm sorry to say that but I tried the attached bt file together with the
extracted media files from the envivio sample and I have still the same
problem (PAUSE IS Not working correctly). 
When I used the mp4tool.exe it outputs following warning:
MP4 Manager  v020318    (c) ENST 1999-2002
WARN:  ESDescriptor -> error fixing ocr idWARN:  ESDescriptor -> error
fixing ocr id
I also tried it with the java version:
MP4 Manager  v020627    (c) ENST 1999-2002
WARN:  ESDescriptor -> error fixing ocr id
WARN:  ESDescriptor -> error fixing ocr id
Because of the reply by Renaud I assume these fixed ocr ID's could be
the reason. There are two OCR_ES_ID's (in the scene description stream
and the ES of the video) refering to an ES_ID 0. But this ES doesn't
exist.
