  Hi John,
    I have experienced problems with QuickTime Player 6.3 while playing MPEG-4 Simple Profile bitstreams "when Data Partitioning, RVLC and/or Resync Marker" are switched ON. Just check if these were ON in the bitstream that you had inserted into .mp4 file, although by looking at the nature of corruption I don't feel this may be the problem.
  Secondly, first try to decode the MPEG-4 compressed bitstream with a Simple Profile Decoder and view the decoded output to ensure that the Encoder has generated the correct bitstream.
