[Mp4-tech] putting MPEG4(H264) ES into MPEG2 TS

Are there any tools anyone knows of to put (H264) ES into MPEG2 TS?
Presumably it has to be put into PES and the leaky buffer model and DPB
buffer taken care of.
Perhaps there is an encoder that inputs from video,YUV or MPEG2 ES direct to
single programme H264 over MPEG 2 transport stream, carrying the AVC?
