Dear Matthew,
 "mb_in_video_packet" is not a syntax element.
It get computed while decoding first group (loop for dc_marker).
You can calculate this by following way.
Just count the number of mcbpc (excluding stuffing type) till
you get dc_marker. This count will represent "mb_in_video_packet"
